Impart 6, The 2019 Debate: Meeting of Minds to Maximize Success Belgrad

February 2019

Prof. Kahraman firstly shared the results of PGD patients obtained from İstanbul Memorial Hospital, ART and Genetics Center titled ‘’Clinical Outcomes in Preimplantation Genetic Screening/Diagnosis for Aneuploidies; What to Do with Mosaic Embryos’’.

Dr. Kahraman, discussed the importance of ovarian reserve and female age in the success of finding at least one chromosomally normal embryo and clinican’s perspective on the prioritizing mosaic embryo.

İn addition, Prof. Semra Kahraman and Prof. Peter Humaidan debated ‘’Freeze All for All Patients’’. Prof. Kahraman reported that freze all with FET cycles may be beneficial in women who are high responders to ovarian stimulation or are at risk of OHSS, but it appears to lose this beneficial effect when applied to women with an intermediate or low response.

She argued that there are in fact no studies proving the advantage of embryo freezing as compared the fresh transfer. The practical obstacles to all freze particularly in low responder patients are high cancellation rate in blastocyst stage all freze, a non- selective freezing can be done which is not ideal for poor responder patients. Limited number of blastocyst stage embryos does not give us the opportunity to select the best candidates for freezing. Another disadvantage is the need for endometrial preparation and furthermore the cost of freezing, storage, FET cycle procedures and embryo pooling.

Finally she stated that all freze of the embryos is the best strategy for reducing the risk of OHSS and provides higher pregnancy rates when compared with fresh transfer. Otherwise, there are no proven advantages in poor responders.

On the last day of the meeting a panel discussion took place, many questions from participants were discussed at the panel attended by  Prof. Semra Kahraman, Prof. Peter Humaidan, Prof. Carlo Alviggi and Prof. Human Hatemi.
